Reservations: In order to ensure that you obtain your desired appointments, we recommend scheduling your appointment as far in advance as possible to ensure availability. To accommodate your desired appointment a credit card is required at the time of booking to guarantee your reservation. We hold this information as part of our cancelation policies and only make necessary charges to your account on the date of your scheduled service. Details of our cancelation policy are provided below.

*Please notify the spa coordinator of any allergies or sensitivities to aromas or ingredients when you schedule your appointment.

Cancellation Policies and Fees: Your spa treatments are reserved especially for you. We value your business and ask that you respect the spa’s scheduling policies. Appointments will be automatically canceled within 72 hours of making your appointment if you do not place your deposit.

Cancel for free up to 12 hours ahead, otherwise you will be charged 40% of the service price for late cancellation or 100% for not showing up. We recognize the time of our clients and staff is valuable and have implemented this policy for this reason. When you miss an appointment with us, we not only lose your business, but also the potential business of other clients who could have scheduled an appointment for the same time. Additionally, many times our staff will be functioning in an “on call” status and can have travelled to the spa specifically for your service. For these reasons we are obligated to compensate our staff for their time as well as make up for the lost revenue.

When you schedule your appointment with us, you are agreeing to these policies. All services require a credit card to guarantee a reservation so please have your credit card ready when booking. You will not be billed unless there is a cancellation or no show. Upon checkout, guests may choose their method of payment.


All New Clients & Clients with a Late Cancel and/or No-Show on file will be required to put a deposit down. (Deposit amount varies based on the service being performed)

Appointments are considered a No Show when there is no contact with an employee by the time of your appointment. 

Prices and Services: Prices and services are subject to change without notice. We will do our best to keep our clients up to date on any changes that may occur but encourage all clients to inquire about pricing and available services when booking your appointment.

Gratuity: Gratuity is not included in the price of service or spa packages, customary tipping runs 15% to 20% but is, of course, at your discretion. You may tip the therapist in the room or upon check out at the front desk with cash, check or credit card. With parties of 3 or more a 20% gratuity will be added to the final bill.

Check in: We ask that you please arrive 10-15 minutes prior to your appointment time. We will do our best to accommodate late arrivals. However, the length of service may be adjusted to not interrupt the scheduled appointments of other guests. Full price of scheduled services will apply, so please plan accordingly. Of course, beyond the check in policies we invite all our guest receiving services to make full use of our relaxing environment here at Himalayan Salt Therapy.

Spa Promotions: Spa specials cannot be used in combination with any other offer, promotions or third part gift certificates.

FINANCIAL POLICY

Thank you for choosing us as your healthcare provider. We are committed to your treatment being successful. Please understand that payment of your bill is considered part of your treatment. The following is a statement of our financial policy, which we require you to read and sign prior to any treatment. Fees: Our fees are determined by the complexity of each case and different services used. Regarding insurance: We will verify coverage prior to treatment and we will file all claims as a courtesy to you. If for any reason we are not able to verify coverage prior to your treatment, you will be charged for the treatment until verification is obtained. We cannot bill your insurance unless you bring us all necessary insurance information. We are not a party to that contract. By signing this document, you are assigning to this office the benefits to which you are eligible to receive for care rendered in this office. Additionally, in signing this document you authorize the release of any information to any insurance company, adjuster or attorney that will assist in the payment of a claim. We request a credit card on file if the insurance company should not pay claims or any balances owed should there be any difference in the amount owed. Usual and Customary Rates UCR: Our practice is committed to providing the best treatment possible for our patients. We charge what is usual and customary for our area. Please be aware that some and at times perhaps all of the services may be non-covered services and not considered reasonable and necessary by medical insurance. All payments are due at the time of service. Missed Appointments: Unless cancelled at least 24 hours in advance, our policy is to charge for missed appointment at the rate of a normal office visit if you are a repeat offender of this rule. Your treatments will be more effective if you follow your physician's guidelines and stick to your treatment schedule. Please help us to serve you better by keeping your scheduled appointments. OFFICE FINANCIAL POLICY AND AUTHORIZATION TO BILL INSURANCE

 There are two billing options available for you. Please select the one you prefer us to use for your visits. If at any time if you choose to change your billing option, you are required to let us know immediately and sign a new Office Financial Policy and Authorization to Bill Insurance Form. Private Pay Private Pay patients are patients that do not bill insurance. This discounted cash rate is only applied to the published rate if you pay at the time of service. Insurance Billing (Medical or Auto Insurance) I understand that I must pay all co-payments and/or co-insurances not covered by my insurance company at the time of check in for today’s visit, and every visit hereafter. Himalayan Salt Therapy will submit my claim for me to my insurance company. Although Himalayan Salt Therapy verifies my insurance; I understand that this verification is not a guarantee of payment. I understand that any and all charges incurred at this office including co-payment, coinsurance, percentage due and/or deductibles or any other fees or services not covered by my insurance company are my responsibility. I understand that if these patient portions due are not paid at the time of service I will be subject to a $10.00 billing fee per month – no exceptions until the outstanding amounts are paid. I further understand that any unpaid balance over 90 days, can and will be sent to collections for recovery unless prior arrangements have been made. I authorize my insurance benefits to be paid directly to Himalayan Salt Therapy. I also authorize the provider to release any information and medical records required by my insurance company. I understand that I may revoke this consent by written request, at any time. No other records shall be released without my signed consent.
